york is used for primarily two things

one you it allows you to appoint an

agent who would be making your medical

decisions

during life if god forbid you are unable

to make them for yourself

and equally important it gives you the

ability to allow your agent to access

otherwise hipaa protected medical

records it’s an extremely important

document and

third

as again a

part of

why a health proxy is important if it’s

properly put in place it limits the need

for guardianship if you ever need a

guardian appointed
